Are you a passionate and proactive nurse looking for an exciting opportunity to make a difference in children's lives? We are seeking a dedicated Children’s Community Nurse to join our Community Nursing Team. This role offers the chance to work autonomously across the borough, delivering holistic, child- and family-centered care for children and young people aged 2–19 years with SEND, PMLD, and complex health needs.
Role Responsibilities:
Assess, plan, deliver, and evaluate clinical nursing care to ensure high-quality and effective patient outcomes.
Develop and maintain detailed care plans to support the safe and effective delivery of care.
Provide specialist nursing care, including but not limited to gastrostomy management, wound care, and suctioning.
Deliver specialist training to families and school support staff to ensure safe and confident care provision.
Work collaboratively with multi-disciplinary teams to ensure continuity of care and attend multi-professional meetings, including child protection case conferences.
Offer emotional and practical support to families, including signposting to specialist services and completing referrals for additional care.
Provide health advice for Education, Health, and Care Plans (EHCPs) to support children's education and well-being.
Minimum Requirements for the Role:
A registered children’s nurse (RNC) with relevant post-registration experience.
Experience in community nursing or working with children with complex health needs is desirable.
Strong communication and interpersonal skills to build trusting relationships with children, families, and professionals.
Ability to work independently while also collaborating effectively within a multi-disciplinary team.
A compassionate, proactive, and child-centered approach to nursing.
